Пациентка 68 лет поступила в плановом порядке с целью выполнения стентирования
Длительно ГБ с АД max = 160/100 мм рт ст, при привычном АД = 110/70 мм рт ст.
СД типа 2 с 2006 г., уровень глюкозы натощак = 7-9 ммоль/л. ДУЗ, эутиреоз.
Со слов около 5 лет беспокоили сжимающие боли за грудиной при усиленных ФН, купирующиеся в покое. Постепенно толерантность к ФН снижалась.
Осенью 2016 г. пациентка обследовалась, со слов было выполнено стресс ЭХО-КГ – тест положительный (мед. документация не предоставлена).
В августе 2017 г. перенесла ОИМ нижней стенки. На догоспитальном этапе возникла АВ-блокада 2 ст. 2 типа, синдром МЭС. Был выполнен тромболиз (Метализе 8000 ЕД). В экстренном порядке выполнена КАГ: в прокимальной трети ПМЖВ 2 тандемные субокклюзии; в проксимальной трети ПКА 2 тандемные субокклюзии с пристеночным тромбом; остальные КА – без гемодинамически значимых изменений.

A 68-year-old female patient routinely admitted for a stenting procedure
Long-term hypertension with BP max = 160/100 mmHg and usual BP = 110/70 mmHg
Type 2 DM since 2006, fasting glucose = 7-9 mmol/L. Diffuse endemic goitre, normal thyroid function.
According to the patient, for about 5 years she has been bothered by constricting retrosternal pain during intense physical exertion, alleviated when at rest. Physical exertion tolerance has gradually diminished.
In autumn 2016 the patient was examined, according to the patient a stress echocardiography was performed – the test was positive (no medical records were provided).
In August 2017 the patient suffered an acute inferior wall MI. In the pre-admission phase a second-degree Type 2 AV block and MAS [Morgagni-Adams-Stokes] syndrome occurred. Thrombolysis was performed (Metalyse 8,000 U). An emergency coronary angiography was performed: 2 tandem subocclusions in the proximal third of the LAD; 2 tandem subocclusions with mural thrombus in the proximal third of the RCA; the remaining coronary arteries showed no significant haemodynamic changes.
