I think I know what hospital this is from, I've done a few of these reports... If I'm right, this is on the upper right hand side of the first page and the context is:
Servei:
Data recepció: [always first]
Data emisió: [always last]
Data tancament: [always second]

In this context, it is NOT the date of discharge, it's the date and time when the lab analysis was completed/the machine finished running. Then the report is issued later (emisió). It couldn't be discharge because how would they be discharging the patient without first issuing the lab results? These times/dates don't have anything to do with the patient's admission or discharge, they refer to when the samples are received, tested, and then results are validated/report is issued by the lab service.
