Catalan to English Medical Medical (general) Medical Report

I was translating a medical report from Spain, and came across a short blurb, which I identified as Catalán...
"Pacient amb MTS de declaració obligatoria, iniciam process d'informe MDO per el ICASS previ informe i comunicació al pacient."

My best guess is:
Patient with ___ required statement?, starts ___ report process for ______ after patient was informed and notified.
