Please rate the comprehensive level of recommendation of the product in a range from 1 to 5 (5 levels).
Does "comprehensive level of recommendation" work? I didn't find many hits in google so I was not confident.

On a scale of 1-5, how likely are you to recommend this product?
"総合" and "度" don't need to be translated literally, and I would not treat this phrase as a fixed expression in English. This is the common way to word this question on a customer satisfaction survey. If you must use explicit mention 総合, I'd use the word "overall."

Satisfaction level
Rather than a literal translation, I suggest "satisfaction level" works here.
"Please rate your overall level of satisfaction with this product from 1-5."
"Please rate your overall level of satisfaction with this product from 1-5."

: In my experience, product satisfaction surveys usually distinguish between satisfaction and likelihood of recommendation.
