Deletion from the Unemployment Register
I think there won't be any bona fide correct way of translating this. In the US at least, I understand unemployed people have to issue claims every week in order to remain on file -- the authority does not decide about whether or not to keep someone on.
Of course all Google searches come up with miserable results, so I call my suggestion only the educated guess of a native English speaker. As long as we are at this level may I suggest
- If the main subject of the form is about the deletion, I think it's okay to leave the word "rozhodnuti" out of the title. Of course, leave it there if the main subject is about how the decision / ruling was made.
- Instead of 'deletion' you could use 'removal', instead of 'register' I think 'records' would also work.
