A pronounced increase in nocturia of 6-7 times. Massive pressing micturition. Cystoscopically, the last control shows a complete bladder neck sclerosis and also recurrent adenomas in the area of the prostate analog. The indication for the transurethral opening of the sclerosis is given.

Disinfection, sterile draping in the lithotomy position. Antibiotic therapy with two ampoules of Bactrim intravenously, team time-out. Entering with the cystoscope to the scleral ring. Impassable. This is now gradually removed with the resection
loop until entry into the bladder is possible. Survey cystoscopy shows unremarkable conditions. No neoplasia, no concrement formation. Orthotopically located ureteral ostia on both sides. Some recurrent adenomas also remain in the area of the prostatic urethra. These are gradually separated so that at the end there is a wide-open resected prostatic analog. **Conclusion with few apical flick steps**.

The procedure is now completed by harvesting the fallen lateral lobes between the 5 and 7 o'clock position, along with the median lobe, if present. The final part of resection can be completed relatively easily as the lateral lobes are already devascularized following separation from the capsule laterally. Deep resection should be avoided at the level of bladder neck to prevent undermining of the trigone. ****Finally, the resection of apical part of the prostate is completed****.
