Quel temps dois je utilisé ici sachant que j'ai utlisé passé simple et imparfait dans mon texte (le debut de cette phrase est au passé simple)?

Son médecin leur avait dit(dit ou disait) que c'était un miracle pour elle d'être enceinte(qu'elle soit enceinte). Je sais que c'est plutot simple comme questions mais quelques fois je me pose vraiment la question de savoir quelle temps temps utilisé dans certains contexte.

si c'est de la littérature, on peut adapter et je trouve que "dire" est un peu plat ici
Quant à la deuxième partie de la question : 'que c'était un miracle qu'elle soit enceinte' me semble très bien, la concordance des temps est respectée
